At TriStar Medical Group, we aim to provide an efficient, friendly and caring environment. We want our facilities to feel as much like your home away from home as possible. To help us better accommodate you, please review the policies below.

Make an appointment

Patients are seen by appointment, and even on the same day the appointment is made, if possible. To schedule an appointment, please call (800) 242-5662 during regular business hours.

Please call in advance for office visits and make follow-up appointments as you leave. We make every effort to stay on schedule, although emergencies do arise. If we are significantly delayed, we attempt to notify patients beforehand.

New patients

Please arrive 15 minutes early to complete paperwork. You can download and print registration forms from our website prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of X-rays, laboratory results and your insurance card to your appointment.

Telehealth appointments

If you prefer to schedule a virtual appointment with your doctor from the convenience of your home, we provide telehealth appointments.

Billing

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards. You can access your records and billing summaries via our patient portal.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and ID with you to every visit so we can ensure that our records are accurate. We recommend that you call your insurance company to verify that we participate with your plan. Please notify us if you have any change in your name, phone number, address or insurance as soon as it occurs. Please present your insurance card at every visit.

Refill requests

We are happy to help with refill requests during regular office hours, so please call your physician’s office to let them know what you need.

Cancellations or rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

After hours and emergencies

If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call (615) 514-0757 and our answering service will assist you.

If you have any questions, please contact your physician’s office or call TriStar Medline at (800) 242-5662.

NOTE: Circumstances may allow for specific exceptions to any visitation restrictions described on this webpage. Those circumstances include religious visitation as well as a designated support person for a patient with a disability to provide assistance with communication or other necessary components of the patient's treatment. All persons entering under an exception remain subject to appropriate infection control protocols.
