Hermitage Primary Care ownership and clinic operations will transition to Summit Medical Group, effective August 1, 2024.
For years, Hermitage Primary Care has proudly served Hermitage and surrounding areas as a community provider of quality healthcare under the HCA Physician Services umbrella.
HCA Physician Services, Dr. Chad Kurzynske and Dr. Ronak Shah have made the decision to transition ownership and clinic operations to Summit Medical Group, effective August 1, 2024.
Summit Medical Group will be the custodian of your confidential medical records as of August 1, 2024 and we may be reached at the same practice telephone number, (615) 232-8812.
If you do not wish to transition care to Summit Medical Group, and would like to identify a new provider, please call TriStar Medline at (800) 242-5662. They will provide a referral to a provider who is located close to where you live or work and accepts your insurance. Any patients who are seeking a change at this time may fax the medical release form found under ‘Patient forms’ to Sharecare at (866) 531-9592.

About Hermitage Primary Care
We are dedicated to providing complete adult primary care services for common, acute and chronic conditions that impact your healthy lifestyle.
Conditions we treat
The internists at our clinic provide internal medicine services to treat conditions and illnesses, such as:
- Asthma
- Chronic obstructive pulmonary disease (COPD)
- Chronic renal disease
- Heart disease
- Hyperlipidemia (high levels of fat particles in the blood)
- Joint injections
Services we provide
Even though we have a family nurse practitioner on staff, we treat only patients aged 18 years and older. In addition to the standard adult clinical services of an internal medicine practice, we offer:
- Chronic disease management for conditions such as diabetes and high blood pressure
- Electrocardiogram (EKG)
- Immunizations
- Laboratory services
- School and work physicals
- Small lesion removals
- Wellness exams for women
We strongly recommend scheduling regular physicals and screenings for cancer and heart disease. We encourage our patients to feel free to discuss their emotional, mental and spiritual issues with us as these factors have a significant effect on your overall health.
In most cases, we can care for your illness or injury at our office. If this is not possible, we will do everything necessary to expedite referrals to other healthcare providers. We will work closely with your specialist to maintain continuity when providing your healthcare.
Arriving for your primary care appointment
Our primary care clinic is located in Hermitage, Tennessee, off of Frist Boulevard. We are part of the TriStar Summit Medical Center campus. We also offer free valet parking.
