We are writing to inform you that The Allen Clinic will be joining Graves Gilbert Clinic, effective August 1st, 2024. Dr. Joseph Allen and Freda Hubbard, APRN are thrilled to continue to serve you and provide the highest level of primary care.
In an effort to maintain consistency in the delivery of care, you can continue seeing the same care team at Graves Gilbert Clinic, located at: 746 Campbell Lane, Suite 101, Bowling Green, KY 42104. Existing patient appointments will transition over to the new practice automatically.
Any patients who are seeking a change at this time may fax the medical release under ‘Patient forms’ to Sharecare at (866) 531-9592.
